MAN OF THE MATCH - ALEKSANDAR MITROVIC
'WE'RE GOING TO WEMBLEY'
The Cottagers will now go on to play the winner of the second play-off semi-final between Middlesbrough and Aston Villa tomorrow evening in the showpiece final on Saturday 26th May.
FT: FULHAM 2-0 DERBY (AGG 2-1)
SIMEON GHOLAM AT CRAVEN COTTAGE
What a performance by Fulham. They simply outplayed Derby throughout tonight and found the two goals they needed. The Rams will be so disappointed that they couldn't hold onto their first-leg lead, but they just didn't have what it took to match the home side here.
Fulham are going to Wembley!
FULHAM 2-0 DERBY (AGG 2-1)
Fulham's play-off curse is over and the Whites are off to Wembley! Ryan Sessegnon and Denis Odoi's second-half goals secure a 2-0 victory for Slavisa Jokanovic's men and they'll play either Middlesbrough or Aston Villa for a place in the Premier League!
Wild scenes at the Cottage as Fulham fans invade the pitch, with Odoi a particular focus of attention...
90+6: Nervy last minutes of the game at the Cottage, but Derby are enjoying much of the possession. Top defending from Ream to kick the ball away from danger but still the Rams come. Keogh nods wide and that could be that...
90+4: YELLOW! Marcus Bettinelli ensures there will be a few extra seconds of the game as he time-wastes and goes into the book as a result.
90+3: WIDE! Derby flood the box with yellow shirts and Palmer threads a ball through for Vydra, but the angle lets him down and his curled effort falls short.
90+1: SUB! Tom Cairney takes an age to leave the field, finding the furthest possible player to hand the armband to, before being replaced by Norwood.
We'll have SIX added minutes here at Craven Cottage...anything still could happen!
90: FREE-KICK! Sessegnon tussles with Weimann and the former goes to ground, but as Kalas races to clear, it is he who earns a free-kick rather than a penalty being awarded the opposite way.
88: OVER! Derby look menacing out of nowhere, but as Nugent works a chance for himself from Vydra's cross-field pass, he miscues an effort from the edge of the box and Bettinelli will have the chance to lump upfield.
86: Fulham are still on the attack here and you wouldn't think that their lead was so slender, such is their confidence on the attack. Sessegnon overpowers a pass to Mitrovic and the Whites concede possession cheaply.
82: The stats show that Fulham have had 10 attempts on target, in comparison to Derby's one...they just need one more goal to level the tie up but are they capable of scoring it?
Fulham are not looking to sit back here by any means. They want to end this game now. Derby have used all three substitutions but haven't shown any real attacking threat since falling behind in the tie. 10 minutes to go...
81: Cairney sets Kamara free on the left and he tries to pick out Sessegnon but Richard Keogh tracks the cross and takes a tumble as he shanks the ball clear.
77: YELLOWS! Booking for Tom Huddlestone as he cynically brings down Mitrovic, before Palmer and McDonald join him in the book after a little disagreement.
76: SUB! Matt Targett's evening is over and he's replaced by Kalas as Jokanovic looks to add some steel to the Fulham defence to see this one out.
75: SUB! David Nugent replaces Cameron Jerome for the remainder of the game. Can he haul the Rams level?
74: YELLOW! Sloppy pass from Ream back to Bettinelli and the stopper can't prevent the ball going behind for a corner. But Fulham break from the corner and Kamara goes down under a supposed challenge from Carson...
Replays show that it was a blatant dive and Carson fumes in the face of the Fulham man as he is booked for simulation.
WATCH: ODOI HEADS HOSTS IN FRONT
Is this the goal that sends Fulham to Wembley?
71: Can Derby bounce back from going behind for the first time over the two legs? Weimann has just headed a great cross from Palmer over the bar, but they've had precious few chances in comparison to the hosts.
However, they still need one goal to take it to extra time as away goals do not count in the play-offs
Fulham with a most un-Fulham like goal. But they won't care a bit! They lead this tie for the first time and now Derby have a quarter of the game to go to save their season.
67: SUBS! Double change from Gary Rowett as he brings on Vydra and Palmer in place of Anya and Lawrence.
GOAL! FULHAM 2-0 DERBY (Odoi, 66) (AGG 2-1)
Fulham are in front! Kamara drifts in from the right and tries to poke goalwards. It's speculative but he wins a corner and from the set-piece, one of the smallest men on the pitch, Denis Odoi, plants a superb header past Carson!
FULHAM 1-0 DERBY (AGG 1-1)
Fulham level in the tie thanks to this man's goal...
64: Another of Anya's crosses from the left flank finds its' way to the grateful palms of Bettinelli. This is a really absorbing tie and it looks as though we could be in for a late one...
62: YELLOW! Johansen has his name taken for a tactical foul on Jerome to thwart a rare Derby break. He's never getting to the ball and that's a good one to take for the team.
61: CLOSE! Lovely close control and turn from Mitrovic to pick up Kamara's pass, but his subsequent shot creeps inches wide of Carson's far post.
60: Our statistical colleagues tell us that Ryan Sessegnon is the first 17-year-old to score in an EFL play-off game since Max Clayton scored for Crewe Alexandra in May 2012...
57: SAVE! Johnson shakes off Odoi and gets into a nice position on the left of the six-yard box, forcing Bettinelli into a smart save with the Rams' first shot on target.
WATCH: SESSEGNON LEVELS THE TIE
You can't keep this man quiet for too long...
54: CORNER! McDonald smashes a deflected effort over the bar after Johansen leaves it for him. The initial corner causes no danger, but Cairney just slightly overhits a pass to Mitrovic as the move comes to an end.
52: The tension has just been ramped up a notch and the atmosphere is superb once more here! But Keogh is on the deck here after a clash of heads with Davies and we'll have a stoppage. That looked very, very nasty.
Sessegnon couldn't take his chance early in the first half, but you don't want to give this young lad two bites at the cherry.
We're all square in the tie now and you'd have to fancy Fulham from here. Chants of "he's one of our own" ring around the Cottage.
GOAL! FULHAM 1-0 DERBY (Sessegnon, 47) (AGG 1-1)
FULHAM ARE IN FRONT! Targett weighs up his options before sending a floated ball into the area. Johansen chests the ball down to Sessegnon and the 17-year-old makes no mistake in smashing into the net from a few yards out! He'd been quiet in the first half but given the chance, he can change a game.
Game on here at the Cottage!
SECOND HALF UNDERWAY! FULHAM 0-0 DERBY (AGG 0-1)
We're back underway at Craven Cottage...
IN-PLAY PRICE BOOSTS
Aleksander Mitrovic to score the game's first goal at 4/1, Ryan Sessegnon to net anytime is 11/2 while Fulham to win by exactly two goals is also 11/2...

WATCH: MITROVIC'S SAVED HEADER
A superb save from Scott Carson, but should the Fulham frontman have done better?
Derby have one man to thank for still being ahead in this tie: Scott Carson. What a stop by the goalkeeper to deny Mitrovic just before half-time, and it wasn't the only superb stop he made in the half.
Fulham have been well on top, but they've not found a way through yet.
FULHAM 0-0 DERBY (AGG 0-1)
The breakthrough hasn't come yet, but my oh my Fulham are knocking on the door. Mitrovic has been at the heart of the action, striking a wicked shot wide on the turn, forcing a one-handed reflex save out of Scott Carson and seeing penalty appeals turned down moments before the break.
Derby haven't really been in the game, but it's approaching the point in the game where you'll wonder if it'll be Fulham's night or not. Stick right here because we've got a big 45 minutes coming right up...
45+1: Will there be one more chance before the interval? We've got just ONE added minute...
44: SAVE! Fredericks opts to drive into the box instead of crossing, pushes Anya all the way back and then sees his shot fingertip saved by Carson!
WHAT A SAVE! They are back at it again just seconds later! Mitrovic gets his best sight of goal this evening, with a close-ranger header, but Carson keeps the ball out with a reflex save with his right hand.
WATCH: MITROVIC'S VICIOUS EFFORT
What a goal this could've been for Aleksandar Mitrovic...
41: SHOT! Bettinelli releases Kamara on the left and he runs the entire length of the pitch before striking a shot that is beaten away by Scott Carson. The former England stopper is really being tested this evening but has coped well thus far.
36: SO CLOSE! Mitrovic picks up Johansen's flick, drops a shoulder, and fires a wicked strike at goal. He beat Curtis Davies very, very well there and is so unlucky to put that left-footed shot wide of the post.
38: CROSS! Anya is delivering some really nice crosses this evening; his latest one finds its' way to Wisdom, but he gets his shot all wrong.
35: SHOT! Sessegnon drives from the halfway line and locates Mitrovic to his right. He passes short to the Serb, but the striker clearly feels pressurised and digs out a shot from distance. It's got very little power and is never going to trouble Carson, though.
