Isaac Vassell is out for "six months" after rupturing a cruciate ligament in Birmingham's draw with Aston Villa on Sunday, Steve Cotterill revealed.
The striker was substituted on the stoke of half-time after colliding with Villa's Alan Hutton, and scans on the 24-year-old's left knee showed damage to his anterior cruciate ligament.
Sam Gallagher replaced Vassell for the second-half of the Second City derby, which finished 0-0 at St Andrew's.
Speaking ahead of Wednesday's visit of Brentford, the Blues manager said: "It's not good for Isaac. It's not good for the club.
"He has ruptured a cruciate ligament."
Vassell is set to see the club's surgeon on Wednesday.
"The learning is put back six months," Cotterill added. "It's a real blow."
