Alan Judge is currently undergoing a medical with Ipswich ahead of a switch from Brentford, Sky Sports News understands.
The two Sky Bet Championship clubs held discussions over the weekend and have agreed terms on a short-term deal.
The 30-year-old midfielder has entered the final six months of his Brentford contract and is set to return to Suffolk.
Should he complete a move to Portman Road, it would see his new home ground become the scene of his devastating leg break suffered in a challenge by Luke Hyam in 2016.
Judge was kept out for a year and nine months after the tackle, which caused multiple fractures and required surgery.
Ipswich have already signed former West Ham defender James Collins on a deal until the end of the season.
The 35-year-old arrived as a free agent after being forced to cancel a short-term deal with Aston Villa last month due to a shoulder injury.
Former Tottenham and Derby winger Simon Dawkins also joined Ipswich as a free agent last week while Huddersfield's Collin Quaner has been brought in on a loan deal until the end of the season.
