The 25-year-old sustained a knee ligament problem during his side 2-1 win over Bologna at the weekend.
Scans have confirmed Inter's worst fears that the Colombian must go under the knife to correct the problem.
The injury could sideline Rivas for up to six months.
The news is a major blow for Inter boss Jose Mourinho who already has Walter Samuel, Ivan Cordoba and Marco Materazzi on the casualty list.
