Eight Championship sides, including Sheffield Wednesday, Ipswich and Birmingham, were beaten in the first round of the EFL Cup on Tuesday night.
Wednesday, who upset Arsenal in the competition last season, were shocked 2-1 in extra-time by League Two Cambridge United at the Abbey Stadium.
Ipswich lost 1-0 at home to League Two Stevenage, with Ben Kennedy's goal shortly after half-time proving decisive, while Liam Sercombe's last-gasp extra-time goal earned Oxford United a 1-0 win at Birmingham.
A late effort from Louis Dodds condemned Huddersfield to a 2-1 defeat at Shrewsbury, while Wigan lost 2-1 at Oldham.
Will Grigg netted for Wigan but Jake Buxton was dismissed shortly afterwards for a high tackle and goals from Ryan Flynn and Josh Law booked Oldham's place in the second round.
Jack Dunn scored a hat-trick as League Two Morecambe won a nine-goal extra-time thriller 5-4 at Rotherham.
Dunn, on loan from Liverpool, completed his treble deep into the additional period after ex-Miller Kevin Ellison had given Morecambe the lead moments before to provide a memorable win.
Brentford lost 1-0 in extra-time at Exeter, with Ryan Harley grabbing the winner, while Northampton came from behind to win 2-1 against Barnsley at Oakwell.
Leyton Orient came up short in their comeback bid against Fulham as the Championship side held on for a 3-2 win at the Matchroom Stadium.
The Cottagers looked to be cruising after Dennis Adeniran's 29th-minute effort and then two goals in three minutes from Cauley Woodrow. However, they endured a nervy finish following Paul McCallum's late brace.
Brighton scored four times in the final 26 minutes to beat visitors Colchester 4-0, with Jamie Murphy netting twice and Sam Baldock and Elvis Manu also on target.
Richard Keogh's goal was enough to give Championship Derby victory at home to EFL new-boys Grimsby.
Nottingham Forest won 2-1 at Doncaster and Eoin Doyle's last-gasp effort gave Preston a 1-0 victory against Hartlepool at Deepdale.
Sheffield United suffered an agonising 2-1 extra-time defeat at home to Crewe, with Ryan Lowe scoring a late equaliser in normal time and then netting the winner in the 100th minute.
Accrington Stanley won a marathon penalty shootout 11-10 against Bradford after the game finished goalless at the Wham Stadium.
Tom Nicholls scored twice, including an injury-time winner, as Peterborough beat AFC Wimbledon 3-2 at the ABAX Stadium.
Goals from Joe Mason and Conor Coady earned Walter Zenga's Wolves a 2-1 win over Crawley at Molineux and QPR loanee Jay Emmanuel-Thomas scored twice as Gillingham won 3-1 at Southend.
Summer signings Joey van den Berg and Roy Beerens scored their first goals for Reading to secure a comfortable 2-0 victory over League Two Plymouth Argyle at the Madejski.
Anthony Stokes netted twice as Blackburn won 3-1 at Mansfield, while MK Dons staged one of the comebacks of the night, fighting back from 2-0 down to beat Newport 3-2 at Rodney Parade.
Wycombe boss Gareth Ainsworth and his assistant Barry Richardson decided against bringing themselves off the bench despite seeing their side lose 1-0 to Bristol City.
Ainsworth, 43, and first-team coach Richardson, 47, were included in the Chairboys' squad at Adams Park due to injuries.
But they watched on from the sidelines as Chelsea loanee Tammy Abraham made it two goals in as many games since joining Bristol City with the solitary goal midway through the first half.
Millwall cruised into the second round with a 4-0 win at Barnet, Carlisle beat Port Vale 2-1 at Brunton Park, Cheltenham were 1-0 winners over Charlton at the World of Smile Stadium and Rochdale saw off visitors Chesterfield 3-1.
Elsewhere, Andy Rose scored the winner in Coventry's 3-2 extra-time victory at home to Portsmouth, Kevin van Veen netted twice in extra-time to give Scunthorpe a 2-0 home win over Notts County and Yeovil and Blackpool won in extra-time against Walsall and Bolton respectively.
