Juve end Real's Euro reign
Real went into the game with a 2-1 first leg advantage but they were never at the races as Juve put them to the sword throughout.
First-half goals by David Trezeguet and Alessandro Del Piero were added to by Pavel Nedved as Juve proved Real are fallible, but many will point to Luis Figo`s missed penalty when the score was 2-0 as the pivotal moment in the match.
Zinedine Zidane scored a late goal to ensure a tense finish, but Juve held firm to set up an all-Italian final with Milan at Old Trafford.
Buoyed by their scudetto triumph, Juve came out fast and sharp proving Real`s superstars are human by forcing them onto the back foot.
Edgar Davids missed the first leg, but it did not take him long to make an impression on Real with a number of crisp challenges.
Davids is known more for his steely tackles than cultured play, but the Dutchman was at the heart of Juve`s good early moves and it came as no surprise when they took the lead.
Real, who almost took an early lead when Roberto Carlos flashed a free-kick inches wide, survived a couple of early scares before a fantastic move cut the Spaniards open.
Pavel Nedved exchanged passes with Trezeguet before crossing for Del Piero whose cushioned header was volleyed past Iker Casillas by Trezeguet.
Real had few chances in the first half, but should have equalised shortly after Juve`s opener. An unmarked Guti shot tamely at Gianluigi Buffon from 12 yards.
Juve continued to push Real back and Casillas was lucky his defenders scrambled the ball away after he spilled a Trezeguet shot.
It came as no surprise when Juve got their second. Del Piero had a quiet first leg in Spain but he came to the party in no uncertain terms on home soil as he dumped Fernando Hierro on his rear with an audacious dummy before slamming a low shot past Casillas.
Real showed greater desire after the break, and the introduction of Ronaldo gave the Spaniards an extra attacking dimension.
Ronaldo provided Real with a lifeline when he won a penalty after tumbling in the box under contact from Paolo Montero.
Figo stepped up but his stuttering run failed to fool Buffon and the Italian international made a comfortable low save.
Juve were under pressure but carried a threat on the counter attack and inspirational Czech ace Pavel Nedved latched onto a fantastic Gianluca Zambrotta pass to rifle a fine shot past Casillas.
The Old Lady were cruising to victory but Raul set Zidane free in the last minute and his low shot beat Buffon and made the aggregate score 4-3.
Five minutes of injury-time had Juve fans on tenterhooks, but Marcello Lippi`s side secured a deserved triumph.
The one sour note for Juventus is that star man Nedved will miss the final after picking up a late booking for a needless foul on Steve McManaman.
