Ismael has agreed a three-year deal at Bayern and will help fill the void left by the departure of Robert Kovac, who seems set to join Juventus.
The move for Frings ends a frustrating one-year spell at Bayern and sees him return to his former club.
Frings spent five seasons at Bremen before joining Borussia Dortmund.
The German international arrived at Bayer last summer from Dortmund with high expectations, but he failed to settle at the Bavarian giants.
Frings has agreed a three-year deal at Bremen and is expected to put pen-to-paper in the next few days.
