The 20-year-old was hurt during the closing 15 minutes of his side's defeat, which confirmed Chelsea's status as Premiership champions for a second successive season.
As Rooney ran towards the opposition penalty area, he fell under a fair challenge from Blues full back Paulo Ferreira.
The former Everton starlet immediately clutched his foot in pain and was in obvious agony on the ground.
He was stretchered off the Stamford Bridge pitch and the immediate observation was that Rooney must be a major doubt for the World Cup finals.
Rooney's Euro 2004 was halted by a foot injury which he suffered early on in the quarter final against hosts Portugal.
The youngster will now be hoping that he will not be forced out of a second consecutive major tournament through injury.
