Roda JC preserved their Eredivisie status after a dramatic penalty shoot-out, while RKC Waalwijk are back in Holland's top division.
Roda JC were four minutes away from relegation to the Jupiler League as Cambuur Leeuwarden led 1-0 in the third match of their promotion/relegation play-off after Rachid El Khalifi's goal in the 35th minute.
But Boldiszar Bodo levelled with time running out before Anouar Hadouir fired Roda in front in the first period of added time, only for Michael Jansen to level from the spot in the 118th minute.
The visitors then held their nerve to triumph 3-1 in a tense penalty shoot-out to retain their top-flight status and leave Cambuur to contemplate another season of Jupiler League action.
Both Leeuwarden scorers lost their nerve from the spot during the shoot-out, along with team-mate Mark de Vries, while Bodo was Roda's only failure.
The two teams were locked together on aggregate after drawing 0-0 in their first meeting and then 1-1 in the second clash.
RKC joy
RKC Waalwijk booked their place in the Eredivisie after a 1-0 victory over De Graafschap in their play-off encounter, which completed a 4-2 success over the three games.
RKC, who finished second in the Jupiler League behind champions VVV Venlo, overcame their opponents who ended the season 17th in the elite division.
Benjamin De Ceulaer scored the only goal of the game when he fired beyond goalkeeper Erik Heijblok to end RKC's two-year absence from Holland's top league.
De Graafschap had lost the first meeting with their second tier rivals 2-0, but won 2-1 on home soil in the next clash, only to fall short in the final showdown.
