McCarthy scored a cracking goal in Palace's 2-1 win at Derby County on Saturday, but also suffered a broken nose in the game.
The defender played the full 90 minutes at Pride Park, but will be assessed by Palace's medical staff to determine whether he can face Southampton.
Alan Lee made a late appearance as a substitute at Derby after more than two months out with a hamstring injury, and he will hope to be involved again, although match-winner Shefki Kuqi should keep his place.
